Veterinary Service Agreement

This agreement outlines the terms and conditions between the undersigned client (referred to as “Pet Owner”) and EveningVets, including veterinary colleagues, associates, and staff (referred to collectively as “Provider”), for the care and treatment of the pet(s) described below. By signing this agreement, the Pet Owner acknowledges and agrees to the following terms:

Terms of Agreement

  1. Consent for Treatment  

The Pet Owner affirms that all treatments, tests, and procedures are conducted with their full consent. No services will be provided against the Pet Owner's will, and the treatment plan has been explained and agreed upon prior to proceeding.

  1. Agreement to Pay for Services 

Payment for all services, including exams, consults, treatments, medications, and diagnostics, is required on the day of service unless prior arrangements have been made in writing. If payment is not received on the same day or within specified grace period, a 4% late fee will be incurred for each month the balance remains unpaid. 

  1. Additional Necessary Treatments 

The Pet Owner acknowledges that during the course of treatment, additional tests or treatments may be deemed necessary by the Provider, including EveningVets veterinary colleagues, associates, or staff, to ensure the health and well-being of the pet. The Pet Owner agrees to be responsible for the costs of these additional procedures, provided the Provider makes a reasonable effort to inform and obtain verbal or written consent before proceeding.

  1. Limitations of Liability 

The Provider will take all reasonable measures to ensure the safety and well-being of the pet. However, the Pet Owner acknowledges that certain risks, complications, or outcomes may arise during or after treatment, including but not limited to adverse reactions, unforeseen medical conditions, or death. The Pet Owner agrees that EveningVets veterinary colleagues, associates, and staff are not liable for any damages, losses, or complications arising from treatments, unless caused by gross negligence.

  1. Emergency Situations  

In the event of an emergency during treatment, the Provider is authorized to take all medically necessary actions to stabilize and care for the pet. The Pet Owner agrees to cover the costs of emergency care.

Acknowledgment and Agreement

By signing below, the Pet Owner acknowledges that they have read, understood, and agreed to the terms outlined in this Veterinary Service Agreement. The Pet Owner confirms that they are the legal owner or authorized representative of the pet(s) listed above.
